About Us

Heartland Health Centers’ mission is to improve the well-being of the communities we serve by providing accessible, high-quality healthcare. We welcome everyone in our communities!
Our holistic approach to health includes primary care for all ages and phases of life, behavioral care, dental, and integrative medicine as well as health classes for patients and neighbors ranging from acupuncture to cooking to Zumba.
We provide care in Lake view East ’s community at HHC-Lake view 3048 N Wilton Ave, Chicago, and HHC-Wilson 845 W Wilson Ave, Chicago , among our 17 community-based, school-based, dental and specialty sites integrated with community mental health centers in the area. We partner with community organizations to put on events such as City Key mobile printing, present at community health fairs, and other activities in the community as well as assist area residents with health insurance enrollment.
We currently provide free COVID-19 testing to people without insurance, regardless of immigration status. Free testing is also available to people with insurance as most health plans cover .testing free of charge.
Heartland Health Centers accepts all Medicaid plans, most PPO plans and some HMOs including Ambetter; we also accept Medicare. We have a sliding fee scale for those that are uninsured; the cost of the visit starts at $30 and includes the consultation, x-rays, and blood work. We offer medication discounts and free interpretation services in over 200 languages.
